Reporting Income

If you are eligible to receive Ontario Works Financial Assistance, you will want to make sure your cheque arrives on time each month!

Tips for Completing Your Card

Be sure to sign and date the card, using a pen, and have your spouse sign as well. It is important that you complete this card in ink, as it is a legal document.

Be sure to fill out the statement of income card correctly.
Read each box carefully. Enter an amount or the word "NIL" in each box, which ever applies. "NIL" means none. Be sure to attach pay stubs or other verification, if you are reporting earnings and/or other income.

Be sure to submit your statement of income card on time!
It should be submitted right on the 16th day of the month, but not before. If your statement of income card is handed in late, or your pay stubs are missing, your cheque may be late.

PLEASE NOTE that if you do not hand in your card, you will not receive a cheque the next month. For each month that you are eligible for income support under Ontario Works, you should receive a statement of income card with your benefit cheque. If you don't get one, please contact your caseworker immediately.

REMEMBER that any change(s) in your circumstances can affect your allowance and, in order to protect your own interests, it is important to let your caseworker know as soon as possible.

Information right at your finger tips! It´s toll-free, convenient and easy to use!

Interactive Voice Response (IVR) is a new telephone system that provides toll-free access to specific personal case file information and general program information about social assistance, using a touch-tone phone. If you´re a social assistance recipient and would like to access this type of information, simply call this toll-free number:

1-800-808-2268

You´ll be asked to enter your 9-digit identification number and 4-digit security personal identification number (PIN). You will then have a number of choices that will allow you to access the information you are looking for.

If you need help using IVR, or if you forget your PIN or ID number, you simply press zero to speak with the Help Desk attendant. The Help Desk attendant is available to assist you from Monday to Friday, 8am - 5pm.
